Job Description

Performs minimally complex social service work. Responsible for authorizing services, oversight of services and evaluating the effectiveness of authorized services. Works under moderate supervision with latitude for the use of initiative and independent judgment. Provides skills/psychosocial rehabilitation to clients authorized for such services. At Center's discretion, may be required to work from the office, in the community, or via telework to effectively accomplish regular case manager job duties. Position may include cross coverage and assignment to serve in other programs to provide treatment services. Examples of work performed

• Case management: Assists in accessing medical, social, educational, and other appropriate services.

Care coordination:

Organizes client care activities and provides safe, appropriate and effective care.



Assesses client's needs and authorizes services to meet the identified needs. Coordinates service delivery.



Coordinates the development of the Person-Centered Recovery Plan.



Provides Crisis Prevention and Management services when needed.



Monitors services delivery. Provides psychosocial rehabilitation to client authorized for services.
