If so, we'd love to meet you. As a Qualified Mental Health Professional (QMHP-CS) , you'll play a key role in supporting individuals on their recovery journey. You'll work directly with patients and their families, helping them access community resources, develop healthy coping strategies, and achieve their behavioral health goals through a compassionate, person-centered approach. In this role, you'll combine care coordination, patient education, advocacy, and community outreach to help improve the overall well-being of the individuals we serve. What You'll Do As a QMHP-CS, you will: Build trusting relationships with patients and their families through ongoing support and encouragement. Complete assessments and develop individualized care plans that support each patient's recovery goals. Ensure services are delivered according to authorized treatment plans and payer requirements. Maintain current assessments, care plans, and authorizations. Provide education using evidence-based behavioral health and recovery-focused curricula. Conduct follow-up visits through phone calls, home visits, and community-based outreach. Use motivational interviewing and goal-setting techniques to empower patients to make positive changes. Connect patients with community resources, transportation assistance, and other supportive services. Provide referrals to behavioral health, social service, and community agencies as appropriate. Coordinate care with healthcare providers and community partners to ensure patients receive comprehensive support. Monitor patient progress and adjust care plans as needed. Manage an assigned caseload while meeting productivity and quality standards. Document patient encounters accurately in the Electronic Health Record (EHR) within 24 hours of service delivery. Maintain HIPAA compliance and confidentiality standards at all times. Keep all required certifications, trainings, and credentials current. Participate in staff meetings, training opportunities, and professional development activities. Perform other duties as assigned. Required Qualifications Bachelor's degree in Psychology, Social Work, Human Services, Counseling, or a related field. Current QMHP Certification. Valid driver's license, reliable transportation, and proof of automobile insurance. Strong communication and relationship-building skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Basic computer skills and experience using Electronic Health Records (EHRs). Knowledge of community resources and social service systems. Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds. Preferred Qualifications Bilingual in English and Spanish. Experience working with behavioral health populations. Experience providing case management, care coordination, or community-based services. Experience working with underserved or high-risk populations.
