•travel to lower valley is a must (Cameron County) & at times McAllen (Hidalgo County)

Duties Include:

Monitoring clients' health statuses who reside in the community. Completing focused assessments & medication monitoring. Assisting & networking with physicians in the community to ensure clients are receiving the best health care possible. Maintaining nursing documentation and submitting weekly service logs. We are seeking a dedicated and compassionate LVN (Licensed Vocational Nurse) / Case Manager to provide comprehensive care coordination for clients with Intellectual and Developmental Disabilities (IDD). The ideal candidate will possess a strong background in nursing, case management, and specialized care for diverse patient populations. The successful applicant will demonstrate excellent communication skills, clinical expertise, and a commitment to patient-centered care. Skills Strong understanding of health regulation policies including HIPAA standards and medical terminology.

Ability to perform comprehensive patient assessments and manage complex clinical case management tasks effectively.

Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across inpatient and outpatient settings.

Experience working with diverse populations including geriatrics, behavioral health patients is a PLUS!

Collaborative environment working closely with team members, case managers. Mostly indoor environment, depending on function needs. Can be fast paced, with frequent interruptions.
