The ideal candidate will have a strong background in case management and a passion for helping individuals navigate their personal challenges. This role involves working closely with clients to assess their needs, develop tailored person centered treatment plans, and provide ongoing support throughout their journey. The Case Manager will also collaborate with other professionals to ensure comprehensive care for clients dealing with various issues, including depression, substance abuse, PTSD, and connections with necessary resources. Duties Conduct thorough assessments of clients' needs and circumstances. Develop and implement individualized case management plans. Provide behavioral counseling and support to clients. Offer addiction counseling services to individuals struggling with substance use. Facilitate individual and group counseling sessions using cognitive behavioral therapy techniques. Collect and analyze data to monitor client progress and outcomes. Coordinate care with other professionals, including healthcare providers and educators. Support clients in navigating inpatient treatment options when necessary. Advocate for clients' needs within the community and healthcare systems. Maintain accurate documentation of client interactions and case progress. Qualifications Bachelor's or Masters degree in human service field (Psychology, Social Work, Counseling) Must meet state requirements for QMHP status (CANS and ANSA Cert, Nurturing Parenting Certification) Experience working with youth or families in a behavioral health setting. Must pass background and fingerprint check. Proven experience in case management or a related field. Knowledge of behavioral counseling, addiction counseling, PTSD care, and special education practices is preferred. Strong skills in data collection and analysis to inform case planning. Ability to conduct individual counseling sessions effectively. Excellent communication skills, both verbal and written. Empathy and understanding towards clients from diverse backgrounds. Ability to work collaboratively within a multidisciplinary team environment. Relevant certifications or licenses in counseling or social work are a plus.
