The ideal candidate will have experience working with adults with long-term psychosis, an understanding of forensic behavioral health systems, and the ability to coordinate comprehensive, person-centered services across clinical, community, and legal settings. Responsibilities Provide case management services to adults with serious and persistent mental illness, including individuals with long-term psychosis. Apply the Collaborative Care Model effectively to coordinate services and promote integrated behavioral health outcomes. Coordinate care and communicate effectively with clients, families, behavioral health providers, community agencies, and other members of the treatment team. Complete ANSA (Adult Needs and Strengths Assessment) assessments and develop, implement, and update individualized treatment plans. Conduct home visits approximately one to two times per month as clinically appropriate and required. Travel between two work locations that are located less than one-half mile apart. Assist clients in navigating community resources, healthcare services, housing, benefits, and other supportive services. Demonstrate familiarity with guardianship programs and mental health courts , including the ability to coordinate services related to court or legal requirements. Maintain accurate, timely, and confidential clinical and case management documentation. Advocate for clients while promoting independence, stability, community integration, and recovery. Collaborate with the treatment team to identify barriers to care and develop practical solutions. Requirements Experience working with the adult mental health population , particularly individuals experiencing serious and persistent mental illness and/or long-term psychosis. Strong knowledge of the Collaborative Care Model and demonstrated ability to implement its principles in day-to-day practice. Experience completing ANSA assessments and individualized treatment plans. Familiarity with guardianship programs and mental health courts . Understanding of the distinct purposes and service requirements of: Psychosocial Rehabilitation (PSR) Case Management Services Medication Training and Support Services Ability to conduct community-based services and home visits. Strong communication, organization, documentation, and care-coordination skills. Ability to work independently while functioning effectively as part of a multidisciplinary team. Reliable transportation and the ability to travel between Bayou Behavioral's two nearby locations and conduct required home visits.
