Overview Chancewell Behavioral Health is seeking a Mental Health Case Worker / Qualified Mental Health Professional-Community Services (QMHP-CS) to provide community-based behavioral health services to children, adolescents, adults, and families in Texas. This is a field-oriented behavioral health position , not a hospital discharge-planning role and not a licensed therapist position. Team members may provide services in clients' homes, schools, approved community settings, the office, and through telehealth when clinically appropriate and permitted. The Mental Health Case Worker will primarily support Mental Health Targeted Case Management (MHTCM) and Mental Health Rehabilitative Services (MHR) by helping clients access needed resources, build functional skills, follow individualized treatment plans, and remain connected to their behavioral health care. Responsibilities Provide Mental Health Targeted Case Management and Mental Health Rehabilitative Services within assigned scope, authorization, treatment plan, and payer requirements. Meet with clients in homes, schools, community settings, the office, and through approved telehealth when permitted. Help clients and families identify needs, barriers, strengths, goals, and available community resources. Coordinate and link clients with behavioral health, medical, educational, housing, social-service, and other appropriate resources. Monitor services and follow up to determine whether referrals and supports are meeting the client's needs. Provide skills training and rehabilitative interventions designed to improve functioning, independence, coping, communication, and community stability. Work collaboratively with LPHAs, therapists, psychiatric providers, schools, caregivers, community organizations, and other members of the client's treatment team. Recognize changes in a client's condition and promptly escalate safety concerns, crises, suspected abuse or neglect, or needs outside the employee's scope. Complete accurate and timely progress notes and other required clinical documentation. Maintain compliance with HIPAA, Texas privacy requirements, Medicaid requirements, payer policies, and Chancewell documentation standards. Participate in required training, supervision, case consultation, quality review, and competency assessments. Maintain required certifications and professional qualifications for assigned services. Requirements Must meet Texas QMHP-CS educational qualification requirements . Bachelor's degree or higher from an accredited institution with qualifying behavioral-health education. Candidates whose degree title does not clearly establish QMHP-CS eligibility may be asked to provide an official or unofficial transcript so Chancewell can verify qualifying coursework. Relevant experience working with individuals experiencing mental health, behavioral, developmental, social, or functional challenges is strongly preferred. Ability to work effectively with children, adolescents, adults, and families from diverse backgrounds. Strong documentation, communication, organization, and time-management skills. Ability to work independently in community settings while following clinical direction and organizational procedures. Reliable transportation and willingness to travel within an assigned service area for community-based appointments. Reliable phone and internet access and ability to use an electronic health record and other secure technology. Ability to complete background, exclusion, credential, education, and compliance verification before providing services. CANS and/or ANSA certification is preferred but is not required at the time of application. Selected candidates must obtain any certifications required for their assigned population and duties before performing services that require them. Important Scope Clarification This position does not independently diagnose mental health conditions, prescribe medication, conduct psychiatric evaluations, or provide psychotherapy unless the individual separately holds the appropriate professional license and has been specifically credentialed and authorized by Chancewell for those services. Prior employment under the title QMHP, QMHP-CS, case manager, behavioral health specialist, or similar title does not by itself establish Texas QMHP-CS eligibility. Chancewell will verify education and qualifications before activation.
