The ideal candidate will be responsible for providing comprehensive case management services to clients, ensuring they receive the necessary support and resources to improve their overall well-being. This role requires a strong commitment to advocacy, crisis intervention, and effective communication with clients. Duties Conduct assessments to determine client needs and develop individualized care plans. Provide ongoing case management services, including monitoring progress and adjusting care plans as necessary. Facilitate access to community resources, including healthcare, housing, and social services. Offer crisis intervention support to clients in distress, ensuring their safety and well-being. Maintain accurate documentation of client interactions, data collection, and case notes in compliance with organizational policies. Collaborate with multidisciplinary teams to coordinate care and communicate client needs effectively. Provide education on addiction counseling, medication administration, and conflict management strategies as needed. Assist clients with home community care services to promote independence and enhance quality of life. Skills Proficient in case management practices and principles. Strong skills in crisis intervention techniques. Knowledge of addiction counseling methods is preferred. Ability to manage conflicts effectively while maintaining a supportive environment. Familiarity with LIHTC (Low-Income Housing Tax Credit) programs is a plus. Experience in data collection and analysis for client assessments. First aid certification is beneficial for addressing immediate health concerns. Competence in medication administration protocols when applicable. Qualifications Master's degree in social work, psychology, counseling, public health, or related field Minimum of 2 years of experience in recovery management, case management, or related field, preferably working with individuals in recovery or with mental health and substance use disorders. Strong knowledge of Medicaid, Medicare, and other community resources. Excellent communication and interpersonal skills with the ability to work collaboratively with a variety of stakeholders. Strong organizational skills and the ability to manage multiple tasks and deadlines. Knowledge of person-centered recovery planning and experience working with individuals in a recovery process.
