At Carease Health we're dedicated to providing exceptional care and support to our patients, and we're seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. As a Social Work/Care Companion, you'll play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. About the Role This part-time position is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. As a Social Work/Care Companion, you'll work closely with our experienced clinical team, conducting assessments, implementing short-term therapy, and documenting visit notes and records. Key Responsibilities Visit designated facilities in accordance with the scheduled set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Qualifications Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills
