Key Responsibilities:
Conduct assessments, care planning, and ongoing case management services Coordinate health and human services to meet individual patient needs Utilize evidence-based assessment tools such as CANS and ANSA Monitor patient progress and support treatment planning Serve as a liaison between patients, providers, and community resources Support quality initiatives aligned with CCBHC, HEDIS, ACO, and other programs Assist with care coordination and patient advocacy efforts Provide support in Behavioral Health Assessment Center operations as needed

Ability to manage multiple priorities in a fast-paced environment Strong communication, organizational, and interpersonal skills Collaborative, patient-centered approach to care Commitment to improving behavioral health outcomes Ability to work effectively with diverse populations Knowledge of crisis intervention principles and behavioral health best practices Strong advocacy skills and attention to detail
Required Qualifications:
Ability to obtain Texas HHS Community Health Worker (CHW) certification within 12 months of hire Ability to complete the Texas Community Partner Program within 12 months of hire Ability to obtain QMHP-CS certification within 2-3 months of hire Bachelor's degree in a qualifying behavioral health-related field, Registered Nurse (RN), or Licensed Practitioner of the Healing Arts (LPHA)
Preferred:
Bilingual in Spanish Background in social work Experience with risk assessments, crisis management, and de-escalation techniques Leadership experience in volunteer or professional settings Ready to Make a Difference?
