Travel Contract Job Summary We are seeking an experienced Social Worker for a 13-week assignment in Layton, UT. The ideal candidate will provide comprehensive social work services, support patients and families, and collaborate with the healthcare team to develop appropriate care plans. Responsibilities Conduct psychosocial assessments and evaluate patient needs. Provide counseling, emotional support, and crisis intervention. Develop and implement individualized care plans. Collaborate with physicians, nurses, and other healthcare professionals. Connect patients and families with appropriate community resources. Assist with discharge planning and coordination of care. Maintain accurate and timely patient documentation. Advocate for patients and help address social and emotional barriers to care. Maintain patient confidentiality and follow facility policies and procedures. Required Certifications BLS Certification
- Required BLS
- AHA•Required LCSW License•Required Must-Have Qualifications & Skills Active LCSW license required.
Previous clinical social work experience preferred. Strong assessment, counseling, and crisis-intervention skills. Excellent communication and interpersonal skills. Ability to work effectively with patients, families, and interdisciplinary teams. Strong documentation and organizational skills. Education Master's degree in Social Work (MSW) from an accredited program. Active LCSW license required. Schedule Monday-Friday | 8:30 AM
