Job Description

DUTIES :
The General Clinical Staff Case Manager will support the ACT with case coordination to ensure the needs of individuals participating in ACT are being met appropriately. These staff members will act as a liaison and report any changes noted in needs to the team. The General Clinical Staff Case Manager will support all aspects of clinical service provision. The General Clinical Staff Case Manager will also function to monitor and support: Individuals with daily living skills-building Connect individuals receiving services with community resources Support attendance at recovery groups Attend and participate in ACT team meetings as scheduled Other duties as assigned by the Executive Director or designee
QUALIFICATIONS
: General Clinical Staff members will, at a minimum, be a Qualified Mental Health Professional, Qualified Mental Health Professional-Eligible, or Qualified Mental Health Paraprofessionals. Possession of a current, valid driver's license and acceptable driving record. Must submit and pass criminal and central registry background checks. Knowledge of the Diagnostic and Statistical Manual of Mental Disorders Knowledge of Community-Based Mental Health Services Understanding of substance use recovery stages of change. Understanding of ASAM Levels of Care Knowledge of mental health and substance abuse interventions. Knowledge of and adherence to basic ethics appropriate for a helping relationship; Knowledge of all Agency Policies and Procedures Effective Communication Skills Ability to Adhere to Documentation Deadlines Ability to work effectively individually and as a team member Ability to support team cohesion
Pay:
$26.07 - $31.39 per hour
