Job Description
Seeking a Senior Social Worker for a full-time position at a new Center for Behavioral Health and Learning, offering a $5,000 sign-on bonus. Partner with patients, families, and healthcare teams to address social, emotional, and medical needs, facilitating treatment, recovery, and safe transitions. Provide psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis interventions, and resource referrals. Coordinate care across settings, ensuring effective resource utilization and comprehensive support for complex cases. Offer specialty consultation, case management, and counseling in specialized programs, adhering to department standards. Complete clinical assessments and develop discharge plans with multidisciplinary teams, ensuring timely and safe patient care. Qualifications include a Master of Social Work, healthcare experience, and Washington state licensure within 90 days.
