A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.
Full Time; Shift Hour and Days vary as needed . Schedules may vary based on program needs and individual preferences.
Typical shifts are as follows:
Morning Shifts:
Start between 8:00 AM•9:00 AM and end between 5:00 PM•6:00 PM, depending on start time and lunch break duration.
Afternoon Shifts:
One required per month, generally 11:00 AM•7:30 PM or 8:00 PM, depending on lunch break.
Weekends & Holidays:
9:00 AM•5:30 PM.
Weekend Requirement:
Staff are required to work one weekend shift per month .
