  • Full-time Job Overview We are seeking a dedicated and compassionate Targeted Case Manager to join our dynamic healthcare team.
In this vital role, you will coordinate comprehensive care plans for individuals with substance use and behavioral health needs, ensuring they receive the appropriate services to promote recovery, stability, and independence. Your expertise will help bridge gaps in care, facilitate smooth transitions across settings, and empower clients to achieve their recovery goals. Responsibilities Conduct thorough patient needs assessments to determine individualized care needs. Develop and implement targeted care plans that incorporate discharge planning, post care coordination, and outpatient services. Collaborate with multidisciplinary teams—including social workers, behavioral health specialists, physicians, and addiction counselors—to ensure holistic patient care. Manage electronic health records (EHR) efficiently while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Facilitate case management intake processes for new clients, ensuring accurate documentation of records and evaluations. Coordinate utilization management and case management activities to optimize resource use and patient outcomes across varied settings. Provide patient service by supporting individuals with disabilities or mental health challenges through personalized case management strategies, including addiction counseling and behavioral health support. Requirements Experience working with electronic health records (EHR/EHR management systems), including documentation of medical records and discharge planning. Certification in Crisis Intervention and CPR required; additional certifications such as HIPAA compliance or managed care credentials are advantageous. Excellent task prioritization skills coupled with the ability to manage multiple cases efficiently while maintaining high-quality patient care standards. Knowledge of resources and excellent networking skills. Experience in targeted case management and Medicaid requirements for TCM services.
MUST HAVE
