JOB:
Seeking an experienced Chaplain with a hospice heart. This is a PRN (part time) position working with patients in the Globe/Miami and surrounding areas. The hospice Chaplain role is to meet the patient on their journey and help provide spiritual support. •Formal Education in social work, seminary, theology or ordained minister is
REQUIRED.
Experience working with death and dying for individuals/family/caregiver preferred. Compensation is paid per visit + Mileage, and hourly rate bereavement calls. Collaborate with healthcare teams to develop and implement care plans that respect patient and family rights and preferences. Conduct regular home visits and telephone outreach to assess the emotional and spiritual needs of patients. Make periodic bereavement calls and share additional resources with families based on the need. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Facilitate memorial services and support groups as needed. Ensure a compassionate and respectful environment for all patients and their families.
