Hospice Spiritual Care Coordinator PRN
Job Description
$65/ patient visit, $30/ administrative hour. The Hospice Spiritual Care Coordinator is responsible for the provision of spiritual care services to patients and families/caregivers of Noble Hospice, either directly or through coordination of care with other spiritual counselors.
POSITION RESPONSIBILITES
Conduct a spiritual assessment as part of the comprehensive assessment within 5 days of admissions to hospice program. Assist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care. Provide spiritual support in accordance with on-going wishes and needs of the patient/ family in accordance with their religious preferences or definition of spirituality. Work in close collaboration with local clergy when desired by the patient Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the plan of care and daily notes Evaluate the patients and family's response to, and effectiveness of, the spiritual counseling Confirm, on a weekly basis, the scheduling of visits with patients' case managers to facilitate coordination of other staff visits Participate in IDG and QAPI meetings and activities Conduct religious services of prayer, worship, and rituals for patients/ families, as appropriate Work with other professionals in resolving spiritual/ ethical issues Provide spiritual and bereavement support to staff as needed Provide consultation and education to the IDG on spiritual care Provide funeral or memorial services for patients as requested Assume responsibility for self-development by continually striving to improve Spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings and/ or individual research and reading Assists with funeral or memorial services for patients as requested. Accepts and performs other related duties and responsibilities as required.EQUIPMENT OPERATION
Use of computer, cell phone telephone, calculator, copier and other office and medical equipment.COMPANY INFORMATION
Has access to necessary patient medical records.QUALIFICATIONS
At least 18 years of age. Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver. Ability to work as member of interdisciplinary group. Comfort in an interfaith setting. Ability to communicate well, both verbally and in writing. Ability to accept different lifestyles, cultures, beliefs, and values. Ability to network with community clergy and congregations. Knowledge of and commitment to hospice philosophy of care. If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Hospice experience preferred. Ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm.Job Type:
