Job Description
Hospice of Marion County, a part of Empath Health is seeking a compassionate, experienced, and dedicated Chaplain to provide spiritual, emotional support and services consistent with the spiritual beliefs of our patients, families, staff and volunteers. Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. As part of Empath Health, we help neighbors with advanced illness live as comfortably and meaningfully as possible, while supporting their families and caregivers with respect, guidance, and dignity. What you'll Do Assess spiritual care needs of patient/families referred for spiritual support. Complete initial spiritual care assessment within the timeframe defined by Center for Medicare and Medicaid Services. Develop an individualized spiritual care plan customized to patient/family goals, needs and concerns. Responsible for scheduled on-call rotations, making visits as requested. Provide bereavement support to patients, families, and team members to support effective grieving. Development and provision of patient funerals and memorial services as well as agency community memorial services. Document patient care activities in the electronic medical record according to agency guidelines. Why Join Empath Health?
