The Care Team Hospice is seeking a compassionate and empathetic Chaplain to provide spiritual and emotional support to patients and families during life's most meaningful moments. This is an opportunity to make a lasting impact by helping individuals navigate their hospice journey with dignity, comfort, and peace. As a Hospice Chaplain, you'll partner with our interdisciplinary team to provide individualized spiritual care, emotional guidance, and bereavement support while honoring each patient's beliefs, values, and traditions. Key Responsibilities Provide compassionate spiritual care, counseling, and emotional support to hospice patients and their families based on individual beliefs, values, and preferences. Complete spiritual assessments, develop appropriate care plans, and document interventions in the electronic medical record. Collaborate with the interdisciplinary team to support patient care planning, symptom management, and bereavement services. Connect patients and families with community faith leaders and spiritual resources when requested or appropriate. Provide ongoing bereavement support and education to families while promoting dignity, comfort, and quality of life throughout the hospice journey. Qualifications Ordained, commissioned, or otherwise recognized and in good standing with a religious denomination or faith tradition. Bachelor's degree preferred; Master of Divinity (M.Div.) or equivalent theological education preferred. Previous experience providing spiritual care; hospice, healthcare, hospital, or pastoral care experience preferred. Knowledge of hospice philosophy, grief support, and end-of-life spiritual care. State Requirements may vary Valid driver's license, reliable transportation, and current automobile insurance required.
