Job Description

Serving St. Joseph & North Missouri Make a Meaningful Difference Every Day At Three Rivers Hospice , we believe exceptional end-of-life care includes caring for the spirit as well as the body. We are seeking a compassionate, faith-centered Hospice Chaplain/Spiritual Care Counselor to provide spiritual guidance, emotional support, and comfort to patients and their families during one of life's most meaningful journeys. What You'll Do As a valued member of our interdisciplinary hospice team, you will: Provide compassionate spiritual counseling and emotional support to hospice patients and their families throughout their hospice journey and during the initial phase of bereavement. Complete spiritual assessments and develop individualized spiritual care plans. Visit patients wherever they call home, offering prayer, counseling, education, encouragement, and spiritual guidance based on each patient's beliefs and wishes. Support patients and families as they navigate end-of-life decisions and the dying process. Coordinate with local clergy, churches, and faith communities when requested. Participate in interdisciplinary team meetings to ensure each patient's spiritual needs are incorporated into their Plan of Care. Communicate spiritual concerns and bereavement risk factors to the Bereavement Services team. Assist families with final arrangements, including connecting with clergy, coordinating religious rites or sacraments, and planning memorial or funeral services. Conduct worship services, funerals, memorial services, or religious rites when requested and appropriate. Qualifications Applicants must meet one of the following: Ordained, commissioned, or credentialed by an organized religious body and have completed at least one unit of Clinical Pastoral Education (CPE); or Master's degree in Divinity or Theology; or Bachelor's degree with an emphasis in counseling or a related field , with completion of required hospice spiritual care training within 90 days of hire, including: Spiritual issues surrounding death and dying Comparative religious beliefs Spiritual assessment Individualized spiritual care Diverse spiritual practices and rituals The Ideal Candidate Compassionate and empathetic Excellent listener and communicator Comfortable serving individuals from diverse faith traditions and belief systems Able to work independently while collaborating with an interdisciplinary healthcare team Passionate about providing dignity, comfort, and hope during end-of-life care Why Join Three Rivers Hospice?
