Job Description

Quality Care Home Health & Hospice - Lakes Region Part-Time | Flexible Schedule Make a difference every day. Provide spiritual support and compassionate care when it matters most. Quality Care Home Health & Hospice is seeking a Chaplain to join our dedicated interdisciplinary hospice team. In this rewarding role, you'll provide spiritual and emotional care to patients and their loved ones facing end-of-life experiences, either directly or through coordination with community clergy and spiritual counselors.
What You'll Do:
Offer spiritual assessments and support to patients, families, and caregivers. Collaborate with the care team to integrate spiritual needs into individualized care plans. Provide direct spiritual counseling and coordinate with community faith leaders. Facilitate funerals and memorial services as requested. Participate in interdisciplinary team meetings and patient care conferences. Educate and support staff and volunteers regarding spiritual care. Assist with recruitment and supervision of spiritual care volunteers. Maintain accurate documentation of spiritual care services.
What We're Looking For:
Graduate of an accredited seminary or theological school (M.Div. or equivalent). Experience working with terminally ill individuals and families. Comfortable working in an interfaith and diverse cultural environment. Knowledge of and commitment to the hospice philosophy of care. Ability to work as part of a collaborative interdisciplinary team. Strong communication and networking skills with community clergy and congregations. Valid driver's license, reliable transportation, and auto insurance. Hospice experience preferred.
