PRN Schedule:
Mon•Fri 8 am to 4:30 pm as needed

As a Chaplain the work you do every day makes a difference in the lives of our patients. Our team gives our patients the greatest gift•the ability to spend enhanced quality time with their loved ones in their preferred environment. As a Chaplain , you will be responsible for the provision of spiritual care services to patients and families/caregivers of hospice, either directly or through coordination of care with other spiritual counselors. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

HOW YOU'LL WORK
You'll provide direct spiritual care to patients, families and caregivers.
MAJOR AREAS OF RESPONSIBILITY
Spiritual Care:
Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. Facilitate the development in the interdisciplinary group plan of care to meet identified spiritual needs.
Administrative:
Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
Customer Care:
Provide for funeral or memorial services for patients as requested. Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
Teamwork:
Provide consultation, education, and support to the interdisciplinary group on spiritual care. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.

Compassionate communicator with a positive attitude Patience is a virtue when working with patients, families, physicians, and coworkers Attention to detail is critical, as is being observant and following directions Comfort in an interfaith setting and acceptable of different lifestyles, cultures, beliefs and values.
REQUIREMENTS
Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred. Experience working with death and dying, individuals/family/caregiver. Knowledge of and commitment to hospice philosophy of care. Hospice/ experience preferred. Valid driver's license and auto insurance in your name as a driver. Capable of all physical demands.
