Assist in coordinating and implementing social services programs for residents Support residents and families in adjusting to facility life and addressing concerns Help develop and maintain care plans that align with resident needs and preferences Facilitate resident and family meetings to provide guidance and assistance Provide resources for financial, emotional, and legal support as needed Assist with discharge planning and ensure a smooth transition for residents returning home or to another care setting Maintain accurate documentation and reports in compliance with facility policies and regulationsQualifications:
