Bella Vita Health & Rehabilitation Center is looking for full time Social Services Supervisor to join our team in providing quality care and a strong work environment for our colleagues. We are conveniently located in Glendale The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state and local standards, guidelines, and regulations, our established policies and procedures, or as may be directed by the Director of Social Services and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Required Qualifications:

An individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology. One year of supervised social work experience in a health care setting working directly with individuals.

Preferred Qualifications:

Experience at a large acute hospital or skilled nursing facility Experience working with EMR PCC Stable work experience performing social services in professional office setting Intermediate Microsoft Office Suite skills: Microsoft Excel (ability to sort, sum, insert bar graphs and use formulas), Microsoft Word (ability to create documents and complete mail merging), Microsoft Outlook (ability to manage email, calendar, and task lists) Bella Vita is looking for talented, caring Social Services Supervisor.
