This role supports adults, including Veterans, individuals with disabilities, and older adults, by helping them access services, resources, and supports that promote independent living and improve quality of life. Key Responsibilities Maintain a caseload of at least 45 active consumers while delivering responsive, person-centered program support. Conduct functional needs assessments to identify service needs, goals, resources, and independent living requirements. Develop, implement, and document individualized support plans and spending plans in collaboration with consumers and community partners. Assist consumers with budget management, service selection, and purchasing approved items that support independent living. Support consumers or their representatives with hiring, training, supervising, and terminating support staff while preserving consumer-directed decision-making. Collaborate with healthcare partners, community-based organizations, and other contacts to connect consumers with appropriate resources. Compensation and Benefits Direct hire opportunity.
Pay range:
$20 to $24 per hour. Recruiter notes list pay at $24.00 per hour.
Annual salary listed:
$49,920.
Schedule:
30 hours per week, full-time, non-exempt.
