Role Summary A Support Coordinator is responsible for implementing Modern Support Service's (MSS) philosophy of Universal Enhancement to improve the quality of life for Support Recipients while ensuring services align with the organization's mission and vision. This role involves recruiting, training, supervising, and developing Direct Support Professionals (DSPs), providing ongoing coaching, performance feedback, and in-service training to strengthen staff skills and service delivery. The Support Coordinator monitors and evaluates program effectiveness by conducting field visits, reviewing documentation, and ensuring that services support each Support Recipient's goals and needs as outlined in their Individual Program Plan (IPP) in coordination with the Regional Center. They conduct assessments, track progress through regular reports, and identify and address barriers that may prevent individuals from achieving greater independence and meaningful outcomes. The position also requires maintaining accurate records, monitoring safety and quality standards, and ensuring compliance with agency protocols and regulatory expectations. In addition, the Support Coordinator communicates regularly with leadership, families, and partner agencies, reports special incidents promptly, and participates in management meetings, trainings, and community networking efforts. They participate in an on-call rotation to address urgent staff or Support Recipient issues and manage assigned caseloads of both staff and individuals. Throughout all responsibilities, the Support Coordinator must demonstrate leadership, professionalism, and strict confidentiality while promoting safe, person-centered supports that enhance independence and well-being of MSS Support Recipients. What You'll Do Leadership and Staff Development Champion the agency's philosophy, Universal Enhancement, and programs and services, and model how they enhance the quality of life of Support Recipients. Demonstrate leadership that advances the Company's Mission and Vision, developing staff skills and competencies Interview and select staff who demonstrate the ability and eagerness to understand, implement, and enhance the agency's philosophy and services; provide ongoing supervision and training to assigned staff Deliver on-site training and development to DSPs to build the skills and competencies that enhance Quality of Life and meet Support Recipients' objectives and needs. Plan, manage, coordinate, and evaluate DSPs' work toward program objectives and Support Recipient outcomes through Coaching Plans, 1:1 meetings, and Performance Reviews. Create and lead Monthly In-Service trainings for DSPs covering the agency's philosophy, protocols and expectations, service delivery developments, Universal Enhancement techniques, and tools for skill development. Create and lead Monthly Team Meetings to review services, protocols and expectations, and issues relating to specific Support Recipients and staff. Support Recipient Services and Quality Assurance Conduct field visits to monitor individuals' program objectives and perform quality assurance monitoring related to Medication Awareness, Home Safety and Cleanliness, Earthquake/Fire/Home Safety, and all matters affecting the personal health, safety, and well-being of Support Recipients. Develop Assessments based on each Support Recipient's current needs and future goals, outlining the specific supports needed to build a life of meaning and value. Coordinate with the Regional Center on the implementation of individuals' IPP objectives. Identify barriers to individuals' success in reaching their IPP objectives and develop plans to overcome them. Documentation and Reporting Monitor service reports and documentation written by DSPs. Complete Semi-Annual and Annual Progress Reports for each individual, specifying supports provided, tools needed to continue progress, barriers to progress, and a plan to help the individual work through challenges toward greater independence. Monitor and report incidents involving alleged abuse, injury, unauthorized absences, hospitalization, APS, police, or other protective agency involvement. Provide clear, timely communication to the Director of Client and Family Services regarding Special Incidents and issues involving staff and Support Recipients. Collaboration and Professional Engagement Attend and participate in all Monthly Management trainings and other mandatory trainings and conferences as directed by the Director of Client and Family Services or Executive Director. Interface and network with public agencies and organizations to expand MSS's public presence and develop collaborations that enhance the agency's services and philosophy. Serve as Support Coordinator On-Call on a rotating weekly basis alongside other Support Coordinators, responding to staff and Support Recipient issues according to company protocol.
