$20.00 - $25.00 per hour Position Summary Hemet Hills Post Acute is seeking a compassionate and organized Social Services Assistant to join our interdisciplinary team. The Social Services Assistant supports residents and their families by helping coordinate services, facilitating communication, assisting with discharge planning, and contributing to a positive resident experience. This role is ideal for someone who is passionate about serving seniors and individuals in a skilled nursing and rehabilitation setting. Essential Duties and Responsibilities Assist the Social Services Director with resident assessments, care planning, and discharge planning activities. Support residents and families by providing information regarding available resources and community services. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Coordinate appointments, transportation, and referrals as needed. Help address resident concerns and support resident rights and quality of life initiatives. Participate in interdisciplinary team meetings and care conferences. Assist with admissions, transfers, and discharge processes. Promote a positive and supportive environment for residents, families, and staff. Maintain confidentiality and comply with HIPAA regulations. Qualifications High school diploma or equivalent required. Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred. Previous experience in a healthcare, skilled nursing, rehabilitation, or social services setting preferred. Strong interpersonal, communication, and organizational skills. Ability to work effectively with residents, families, and healthcare professionals. Proficient with basic computer applications and electronic health records preferred. Ability to maintain professionalism, empathy, and confidentiality in all interactions.Benefits Competitive pay:
