A Social or Human Services Assistant assists with identifying and arranging community services for clients. Works with social workers and other human services professionals. Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.
Job Summary Brian Gibbons Homeless Outreach (BGHO) is seeking a compassionate, driven, and resourceful Street Outreach Worker to join our ROST (Reentry Outreach and Stabilization Team). This position will primarily work with individuals experiencing homelessness or housing instability who have a history of incarceration or involvement with the criminal justice system. The Street Outreach Worker will meet individuals where they are -on the street, in shelters, in the community, and within partner systems and provide consistent, housing-focused support. The ideal candidate will be comfortable working independently in the field, building trust with individuals who may be hesitant to engage with services, and developing strong partnerships across the criminal justice, housing, employment, and social service systems. Duties Street Outreach & Engagement Conduct regular street outreach to individuals experiencing homelessness and housing instability. Reentry & Criminal Justice Partnerships Develop and maintain strong working relationships with police departments, probation/parole, courts, correctional reentry programs, and other criminal justice partners. Attend police roll calls and other relevant meetings to identify individuals who may benefit from outreach and reentry services. Establish referral relationships with courts, reentry programs, correctional facilities, and community organizations. Coordinate services and advocate for clients navigating the criminal justice system. Support individuals returning to the community following incarceration with their transition toward housing, employment, and stability. Housing & Case Management Assist clients in identifying and securing safe, affordable, and sustainable housing. Develop and maintain relationships with landlords and property management companies. Advocate with landlords and housing providers on behalf of clients facing barriers. Assist with housing applications, documentation, housing searches, and housing stabilization. Connect clients with shelters, transitional housing, supportive housing, and permanent housing resources. Develop individualized service plans and assist clients in achieving identified goals. Benefits & Employment Assist clients with applications for SNAP, Medicaid, SSI/SSDI, and other benefits. Help clients obtain identification and other critical documents. Connect clients with employment programs, workforce development agencies, and educational opportunities. Assist with job searches, applications, transportation planning, and employment retention. Connect clients to medical, behavioral health, substance use, and other community-based services. Documentation & Collaboration Maintain accurate and timely case notes and client records. Complete required HMIS and ROST program documentation. Track housing, employment, benefits, and other client outcomes. Participate in team meetings, case consultations, trainings, and community meetings. Collaborate with BGHO staff and community partners to provide coordinated services. Build trusting, long-term relationships with individuals with histories of incarceration. Provide low-barrier, trauma-informed, and person-centered support. Respond to immediate needs including transportation, identification, food, clothing, hygiene supplies, and other basic needs. Provide crisis intervention, de-escalation, and problem-solving support. Requirements Experience in street outreach, homelessness, reentry, case management, criminal justice, or social services strongly preferred. Experience working with individuals who have a history of incarceration is highly valued. Strong understanding of barriers to housing, employment, and community reintegration. Excellent communication, organization, advocacy, and problem-solving skills. Ability to build professional relationships with clients, landlords, law enforcement, courts, and community partners. Ability to work independently in the community and respond effectively to changing situations. Ability to maintain professional boundaries while developing trusting relationships. Valid driver's license and reliable transportation required. Preferred Bachelor's degree in social work, human services, criminal justice, psychology, or a related field preferred.
