Live Oak, Florida Schedule:
Full-Time | 8:00 AM - 4:30
PM Salary:
$23-$26 per hour, based on experience Join Our Social Services Team! Live Oak Healthcare and Rehabilitation is seeking a compassionate, organized, and dedicated Social Services Assistant to support our residents and their families. This position plays an important role in helping residents navigate their healthcare experience, connecting them with resources, and supporting their emotional and social well-being. Key Responsibilities Conduct resident and family interviews to gather social histories and identify individual needs. Assist with discharge planning and coordinate appropriate community resources and services. Support the development and implementation of individualized social service and care plans. Participate in resident assessments, care planning meetings, and departmental programs. Provide compassionate support and customer service to residents and their families. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Report occupational exposures in accordance with facility policy and procedures. Participate in emergency drills and carry out assigned responsibilities under the facility's emergency preparedness plan. Stay current on industry best practices and incorporate effective approaches into social services programs. Collaborate with the interdisciplinary team to promote quality resident care and outcomes. Qualifications Minimum of 1 year of social services or social work experience in a healthcare setting . Skilled Nursing Facility (SNF) experience preferred. Experience with Medicaid in Long-Term Care (LTC) or Skilled Nursing Facility settings preferred. Knowledge of Code Statuses preferred. Experience with PCC (PointClickCare) preferred. Strong communication and customer service skills, particularly when working with elderly populations. Currently pursuing a degree in Social Work, Human Services, or a related field preferred. Previous social services experience and relevant education are a plus.
