The Case Management Assistant responsibilities include: Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care Responsible for assisting with distributing regulatory documents to identified patients Educating patients about the appeal process related to their discharge Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives Position details:

Education:
Required High School or Equivalent Preferred Associate's Experience:
