Monday-Friday 9:00 AM-5:30

PM Key Responsibilities:

Develop and implement a comprehensive social services program for residents and families Provide counseling and address medical-related social needs Coordinate and facilitate family meetings and ongoing communication between residents, families, and the care team Advocate for residents' rights and serve as a liaison among all stakeholders Manage discharge planning and transitions of care Deliver exceptional service in a supportive and respectful environment

Qualifications:
