Job Description

Come Join Our Team The Social Services Designee supports the overall well-being of residents by completing required assessments, maintaining accurate psychosocial documentation, supporting care planning and discharge planning, and ensuring resident rights and regulatory requirements are communicated and documented. This role works closely with residents, families, and interdisciplinary team members to assist with admissions, transitions, room changes, and ongoing social service needs within the assigned area.

Qualifications:

Completion of Social Services Designee program (48 hours) and certification.
