This position works under the direction of the Administrator and Director of Social Services. Key Responsibilities Implement the facility's social services program Assess resident social service needs and coordinate resources Assist residents and families with discharge planning and community referrals Coordinate care with the interdisciplinary team (IDT) Participate in care planning, MDS social service components, and resident councils Provide education on resident rights, advance directives, and available services Assist with admissions, transfers, and discharges Maintain compliance with state and federal regulations Qualifications Bachelor's degree in Social Work or related human services field Minimum one (1) year supervised social work experience in a healthcare setting. Candidates who have completed their social work internship are encouraged to apply. Strong communication and documentation skills Ability to work effectively with residents, families, staff, and community agencies Physical Requirements Ability to lift, push, or pull up to 25 lbs Ability to move throughout the facility and assist during emergencies as needed
