Job Description

Job Overview We are seeking a dedicated Program Support Coordinator to oversee and manage multiple programs effectively along side the Program Director and Assistant Program Director. The ideal candidate will have a strong background in health care, social work, program management, and administrative tasks. Responsibilities 1. Assist in onboarding and training new Family Home Providers and Direct Support Professionals. 2. Attend transition meetings and coordinate smooth transitions. 3. Complete initial home monitoring. 4. Conduct home visits and provide additional support to distant, new, and high-need Family Home Providers. 5. Maintain Personnel records 6. Inform supervisor of any changes requested by the Waiver Recipient's (SCL & MPW) or guardians or team in the level of services being received and also coordinate authorized changes to the program. 7. Assist with transportation when necessary. 8. Communicate with all other agencies or family member/guardians involved with each Waiver Recipient (SCL & MPW). 9. Ensure compliance as required with funding sources i.e. Supports for Community Living Waiver and Michelle P. Waiver. 10. Know and understand the regulations and how to comply with them. 11. Assist in all aspects of programming. 12. Work or fill in shifts as needed. 13. Maintain and enforce appropriate boundaries between staff and between staff and Waiver (SCL & MPW) and Non-Waiver Recipients. 14. Educate staff on different aspects of treatment, including conducting and documenting in-services. 15. Train and educate employees of the types of behaviors prohibited and inform employees about Finck Supported Living Services expectations. 16. Assist in the prevention of unethical behavior by: 1) refraining from participation in or encouragement of actions that could be perceived as unethical 2) reporting to a supervisor any unethical behavior. 17. Fully cooperate in any investigation. 18. Supervise Family Home Providers and Direct Support Professionals. 19. Know and carry out the policies of Finck Supported Living Services. 20. Will perform other duties as assigned by the Program Director or Executive Director. Experience
  • Experience in supervising teams and managing programs
  • Strong background in heath care, social work or related field
  • Proficiency in budgeting and financial management
  • Excellent relationship management skills
  • Experience in working with individuals with developmental disabilities is a plus
  • Demonstrated ability in program development and administration

$30,000.00
  • $37,000.
