Position Summary The Support Coordinator Supervisor is responsible for supervising and developing Support Coordinators while ensuring quality services, regulatory compliance, and person-centered practices. The Supervisor also conducts field visits, participates in quality assurance activities, assists with staff development, and serves as a resource for both employees and the individuals receiving services.
Minimum Qualifications Required:
Bachelor's degree in a Human Services-related field, including: Psychology Sociology Social Work Counseling Education Child and Family Studies Criminal Justice Human Services or another closely related field Minimum of two (2) years of post-graduate experience providing case management/support coordination services Strong leadership, communication, organizational, and problem-solving skills Ability to effectively coach, mentor, and motivate staff Valid driver's license, reliable transportation, and current automobile insurance
Preferred:
Previous supervisory or leadership experience Experience with Louisiana Medicaid Waiver Services and Support Coordination Knowledge of Person-Centered Planning principles and Louisiana Department of Health regulations Essential Responsibilities Supervise, mentor, and support a team of Support Coordinators. Work collaboratively with the Program Manager to oversee daily operations. Teach, model, and reinforce Person-Centered Planning principles and practices. Provide ongoing coaching, training, and professional development. Conduct regular supervision meetings and performance evaluations. Review documentation, service plans, case notes, and participant records for quality and compliance. Ensure agency timelines and state documentation requirements are consistently met. Monitor caseload assignments and staff productivity. Conduct home visits and accompany staff in the field to provide coaching and quality oversight. Assist Support Coordinators with complex participant needs, crisis situations, and problem-solving. Ensure compliance with Louisiana Medicaid Waiver regulations, agency policies, and quality assurance standards. Coordinate and monitor required staff training and continuing education. Maintain accurate supervisory records and documentation. Participate in quality improvement initiatives and performance monitoring.
