GENERAL SUMMARY
Under the supervision of the Senior Center Director, develop and manage the programs and activities of the Holden Senior Center. Assist in the development and management of the programs, activities and services of the Holden Senior Center.
ESSENTIAL JOB FUNCTIONS
- : • Assist in overseeing such peer support groups as Friendly Visiting, Telephone Reassurance and other groups as needed.
- Identified frail elderly, assess their needs and provide needed service whenever possible.
- Makes referrals to proper agencies provides follow-up, assists with support groups.
- Works with other Outreach Worker and Director to maintain Outreach programs. (Book Express, Food Pantry delivery, etc.) Seeks volunteers to help programs function.
- Works with other Outreach Worker, Director and HPD to maintain Telephone Reassurance program and follows up on needs that arise.
- Conducts in-home visits to elders as needed. Assesses needs, determines course of action to meet those needs and follow-ups to ensure adequacy of assistance.
- Keeps statistical records on all services and programs provided. Keeps hard copy records on individual clients as well as inputs statistics into the My Senior Center software.
- Researches, gathers information and reviews applications for Help at H.O.M.E. Seeks approval from sub-committee that oversees the fund and makes certain moneys are expended for each participant through the appropriate channels.
- Initiates contact with seniors using the listing of seniors as generated by the town clerk's office as well as referrals from other sources.
- Assess and facilitate connections to services and programs for elders who may be un-served or underserved.
- Conducts home and/or office visits independently to assist elders in assessing their needs and to facilitate access to services. Also, works with families as necessary.
- Interacts with the Director on a regular basis for specific case management, to share concerns, and to arrange appropriate additional services.
- Able to differentiate between basic client needs and those of a more serious nature. Can identify an emergency, call 911, and inform the Director.
- Responds to crisis; coordinates with local agencies and safety departments to modify or alleviate crises.
- Assists clients in applying for financial support, i.e., fuel assistance, food stamps, MassHealth, and tax exemptions.
- Maintains regular contact with elder service agencies and networks to coordinate client care.
- Monitors changes in eider's situation and provides follow-up assistance.
- Documents identified needs and client status.
- Tactful with ability to respect and maintain confidentiality.
- Good social/inter-personal communication skills.
- Thorough knowledge of and sympathy with problems of elders.
- Some experience or exposure to group work.
- Maintains confidential client files
- Attends relevant seminars and training courses.
- Maintains and promotes Telephone Reassurance program, Volunteer Opportunities and Support Groups.
SKILLS/EXPERIENCE/TRAINING REQUIRED
- Excellent administrative skills
- Ability to listen and observe
- Tactful with ability to respect and maintain confidentially
- Good social/inter-personal communication skills
- Thorough knowledge of and sympathy for issues facing senior citizens
- Some experience or exposure to group work
- Computer skills (experience with Microsoft Office preferable
- BA in social work or counseling preferable
SUPERVISORY RESPONSIBILITY
- Assists in supervision of outreach volunteers
WORKING CONDITIONS
- Work is performed primarily indoors in an office or home environment.
- Occasional walking, standing, lifting and climbing is required.
- Operates a variety of office equipment including computers requiring eye-hand coordination and finger dexterity.
- Valid Massachusetts driver's license required
- Travel in personal vehicle is required. Mileage reimbursement available.
