Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.

Job Description
- Develop and implement individual plans of service using the person centered planning process for residents choosing to participate with supportive services.
- Create a welcoming environment for all residents.
- Facilitate and advocate for access to community resources and property management for accommodations as necessary.
- Assist with benefit acquisition and maintenance including Medicaid, Medicare, Social Security, DHHS Home Help, food assistance, MSHDA Section 8, etc.
- Facilitate transition processes including arrangements for follow up services.
- Utilize Motivational Interviewing to encourage movement through stages of change with resident clients.
- Collaborate and coordinate services with primary case managers, when applicable, including obtaining a copy of updated/current individual plans of service.
- Perform comprehensive assessments to determine medically necessary services.
- Encourage residents to participate in needed support services on a case by case basis.
- Coordinate and assist in crisis intervention and stabilization as needed, using a 24 hour on call system.
- Provide coordination of care with various supports for each person served.
- Complete and maintain required documentation.
- Understand and fulfill requirements of existing contracts or Memorandums of Understanding with community, state, or federal agencies as directed.
- Relay maintenance or safety concerns to property management staff.
- Assist with grant renewals and contract development through record keeping and reporting.
- Establish a frequent presence at assigned housing sites.
- Mentor or coach residents to develop competencies and skills to increase and enjoy social support networks and stress management/distress tolerance.
- Organize and facilitate resident activities focusing on developing group interactions and leadership skills.
- Provide residents with information on transportation options and availability.
- Facilitate volunteer, vocational, and employment development through referrals and on site opportunities.
- Support and promote a recovery focused environment.
- Assist property management with resident selection, stability, and eviction processes.
- Facilitate resident participation in property management, resident organization, educational, and safety presentations.
- Consistently provide quality customer service in an inclusive and friendly manner.
- Collaborate on case consultations with other staff as needed.
- Perform other duties as assigned. . Supervisory Responsibilities The Resident Supports Coordinator may have supervisory responsibilities for:
- Student Interns
- Volunteers
- Recovery Coach . Required Qualifications
- Bachelor's degree in Human Services or a related discipline. Bachelor of Social Work preferred.
- Experience working with diverse and vulnerable populations, including the aged, those with developmental disabilities, severe and persistent mental illness, substance use and/or co occurring disorders is strongly preferred.
- 3 to 5 years of progressive related experience strongly preferred.
- Experience or knowledge of property management and supportive housing preferred.
- Knowledge of community resources for case management of specialized populations preferred.
- Computer literacy with ability to learn new software.
- Strong communication skills.
- Excellent organizational skills.
