Social Services Designee Position summary The Social Services Designee interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. They are responsible to market the services we provide and establish relationships with all referral sources. They clarify a range of legal documents for residents and families as well. As a member of the interdisciplinary care planning team, the Social Service position identifies the psychosocial strengths and needs of each resident, assists in developing an individualized plan of care, and provides medically related social services to help the resident attain or maintain the highest level of functioning possible. Responsibilities Admission and Discharge Coordination Manages the full admission process, including referrals, pre-admission screenings, paperwork, and financial applications; develops and documents ongoing discharge plans and coordinates home health or equipment needs. Resident and Family Support Provides emotional support, guidance, and information to residents and families, assists with adjustment to facility placement, grieving, and advanced directives, and addresses psychosocial needs. Communication and Liaison Acts as a link between residents, families, staff, and community agencies; communicates changes such as room assignments and coordinates care with appropriate personnel. Care Planning Participation Completes social assessments, PHq-9, BIMs, MDS, and care plan documentation; participates in interdisciplinary care planning and patient care conferences to ensure psychosocial and behavioral needs are addressed. Professional Conduct and Relationship Management Maintains courteous, cooperative, and professional relationships with residents, families, staff, and visitors; adheres to dress code, confidentiality, and facility policies. Facility Compliance and Education Promotes QAPI initiatives, maintains accurate departmental records, follows Abuse Prohibition Plan and regulatory requirements, attends continuing education, and keeps current on policies, procedures, and Residents' Bill of Rights. Qualifications Must recognize and understand the psychosocial and emotional needs of residents. Must have strong skills in communication, organization, management, and leadership Must be able to perform essential functions of the position with or without reasonable accommodation.
