Job Summary:

Lakehouse Healthcare and Rehabilitation is seeking a compassionate, motivated, and licensed Assistant Director of Social Services to join our long-term care team. This position plays a key role in supporting residents and their families by ensuring social, emotional, and psychosocial needs are met while assisting the Director of Social Services with daily departmental operations. The ideal candidate will have experience in long-term care and a strong passion for advocating for residents and coordinating supportive services.

Responsibilities:

Assist the Director of Social Services with managing and coordinating the Social Services department. Support residents and families with adjustment to the facility and ongoing psychosocial needs. Conduct resident assessments and assist with care planning in accordance with federal and state regulations. Provide counseling, guidance, and support to residents and families.

Qualifications:

Coordinate discharge planning and community resource referrals. Must hold a current and active Social Services license as required by state regulations. Prior long-term care or skilled nursing facility experience preferred . Strong communication, organizational, and interpersonal skills. Ability to work collaboratively with an interdisciplinary team. Knowledge of state and federal regulations related to long-term care social service Advocate for residents' rights and ensure compliance with regulatory standards. Participate in interdisciplinary team meetings and care conferences. Maintain accurate and timely documentation in the medical record.

Pay:

$55,000.00 - $65,000.00 per year
