Community Springs Healthcare Facility (CSHCF) is a 105-bed long-term care skilled nursing facility providing transitional or long-term care. A 15-bed high-functioning memory care unit provides specialized care for residents with Alzheimer's or other memory issues.
A typical day includes:
As a Social Services Designee, you will assist with admission/discharge planning. You will complete regulatory paperwork, assist with family/resident concerns/needs. The Social Services Designee also coordinates the transportation of residents to appointments. You will work to utilize the communities' resources to assist in the discharge of short stay residents. Delivering resident mail the same day as received Assessing each resident's psychosocial status Serving as a liaison between the families of residents and the health care team in time of crisis Making arrangements for obtaining needed adaptive equipment, clothing, and personal items
Job Requirements:
Education:
High school diploma, MHCA Designee Training required
Experience:
1 year of long term care/healthcare preferred
Skills:
Collaborative, detail oriented, problem-solving
Licensure/Certification:
Family and Friends CPR within 6 months of employment
Physical:
