NO EXPERIENCE OR LICENSE REQUIRED. IN HOUSE TRAINING FOR THIS POSITION
Responsibilities Conduct thorough resident assessments Assist residents in discharging back to the community by setting up appointments,home health,etc Scheduling appointments for residents. Deliver culturally competent social services by working effectively with diverse populations and understanding their unique backgrounds. Support residents with case management tasks such as developing action plans, documenting progress, and coordinating services across agencies. Maintain accurate records through clear documentation of resident interactions, care plans, grievances and incident reports while adhering to confidentiality standards. Collaborate with multidisciplinary teams including healthcare providers, social workers, and community organizations to optimize resident outcomes. Support geriatric patients by addressing their specific needs related to aging services and disability accommodations. Coordinate with families and other departments for care plan conferences. Marketing once a week at hospitals,home health and hospice agencies locally. Qualifications. Excellent client communication skills combined with active listening abilities to foster trust and rapport. Ability to work independently while collaborating within a team environment in fast-paced settings. Eager to learn the role of social services.
