Job Description
Continental Care and Rehab is looking for a team member to join our social service department. The preferred candidate will possess a license in social service, equivalent background and experience will be considered. We are a 100 bed long-term care skilled nursing facility that prides itself in the quality of care in service to those special individuals we serve. The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and assisting with the overall operation of our facility's Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, and our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
