The Social Services Assistant supports the psychosocial well-being of residents in a Skilled Nursing Facility (SNF). This role assists with care planning, resident and family support, discharge planning, and coordination of services to help residents adjust to the facility and maintain quality of life in compliance with state and federal regulations. Essential Duties and ResponsibilitiesResident Support Assist residents with social, emotional, and adjustment needs related to illness, disability, or placement in the facility Conduct and document psychosocial assessments as assigned Provide emotional support and counseling under the direction of the Social Services Director Monitor residents for signs of depression, anxiety, abuse, neglect, or psychosocial distress and report concerns appropriately Care Planning Participate in interdisciplinary care plan meetings Assist in developing, implementing, and updating individualized care plans Document social services interventions accurately and timely in the medical record Family & Resident Relations Serve as a liaison between residents, families, and facility staff Assist with family meetings and address concerns or grievances Educate residents and families on facility policies, resident rights, and available resources Discharge Planning & Coordination Assist with discharge planning, including referrals to community resources, home health, hospice, or long-term placement Coordinate with external agencies to ensure continuity of care Maintain communication with residents and families regarding discharge plans Compliance & Advocacy Support compliance with federal, state, and local regulations (including CMS and Resident Rights) Assist with grievance tracking and resolution Advocate for residents' rights, dignity, and autonomy Administrative Duties Maintain accurate documentation and reports Assist with audits, surveys, and inspections as needed Perform other duties as assigned by Social Services Director or Administration QualificationsEducation & Experience High school diploma or equivalent required Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred Previous experience in a Skilled Nursing Facility, long-term care, or healthcare setting preferred Skills & Abilities Strong interpersonal and communication skills Ability to work effectively with elderly and disabled populations Knowledge of psychosocial needs of long-term care residents Ability to maintain confidentiality and professional boundaries Strong organizational and documentation skills Compassionate, patient, and resident-centered approach Physical & Work Requirements Ability to sit, stand, walk, and move throughout the facility Ability to lift or assist with light lifting as required Ability to work in a healthcare environment with exposure to residents with various medical conditions Work Environment Skilled Nursing Facility / Long-Term Care setting Fast-paced, team-oriented healthcare environment
