A Social or Human Services Assistant assists with identifying and arranging community services for clients. Works with social workers and other human services professionals. Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.

Job Description
Responsibilities:
- Review referrals for admission
- Review insurance and verify benefits
- Meet with families and conduct facility tours
- Build relationships with hospitals, social workers, and discharge planners
- Maintain an accurate facility census and knowledge of available beds
- Help residents and family members to complete paperwork as needed
- Assist Marketing and Admissions Director with tasks as needed
- Assists with discharge, admissions planning and other case management duties
- Clerical duties
Qualifications:
- Bachelor's degree in Social Work, Psychology, Human Services or other related field
- Previous experience in long-term care, healthcare preferred
- Strong communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Ability to prioritize and execute quickly in a fast-paced environment
- Proficient in documentation and record-keeping practices
