BA/BS in human services or related field and two (2) years' experience in disability-related services preferred. Assist consumers with all types of disabilities who have low income, are near homeless or are homeless, and need assistance to find or keep appropriate housing and to live independently in the community. Provide consumers with advocacy, independent living skills, peer support, and information & referral, and conduct outreach. Knowledge of community resources for persons with disabilities a plus. Highly organized individual with commitment to consumer control and empowerment sought. Ability to work in a team environment. Strong communication, customer service, and Microsoft Office skills required. Some local travel required. College course work and/or experience in services for persons with disabilities. 2.

More than two years of experience in service provision for persons with disabilities, strong working knowledge of community resources for persons with disabilities. :

Personal life experience with disability and/or familiarity with the history of the Independent Living Movement. :



Clearly demonstrated adherence to the Independent Living philosophy, best practices, and language usage that support consumer empowerment and self-determination. Articulate with strong written and verbal communication and writing skills. Strong organizational and follow-up skills. Relates to persons in a way that promotes self-determination. Knowledge of CIL core services for persons with disabilities. Knowledge of computers, word processing, and email required. Bi-Lingual (English and Spanish). : The candidate must be able to complete all requirements of the job with or without reasonable accommodation: These include, but are not limited to, accessing and providing support in the community; working with a laptop, phone, and other office equipment with various software programs; sitting, bending, kneeling, lifting (less than 10lbs.) as needed to support work/consumers. III.

Essential Job Functions Core/Housing Specific:

1. Meet with consumers for intake and CDBG income verification at LVCIL offices, community locations, at consumer homes throughout the Lehigh Valley, or through virtual platforms as needed. 2. Educate consumers on housing options. 3. Assist homeless, near homeless, and other consumers with search for housing, including Section 8 applications, apartment applications, contacts to private landlords, and contacts to housing authorities. 4. Advocate for housing placements to occur and/or to help consumers retain appropriate housing. Provide case management to consumers throughout housing transition. Provide follow-up contacts with consumers. 5. Make preliminary visits and contacts to housing options on behalf of consumers. 6. Provide ongoing support to consumers; facilitate referrals to other services as needed; coordinate consumer services with Core & Housing or other LVCIL team members. 7. Provide Independent Living Core Services as needed to consumers including advocacy, peer support, independent living skills education, and information & referral. 8. Provide services on an individual or group basis as needed. Support LVCIL consumer groups. 9. Provide outreach contacts to community organizations; participate in related community groups. 10. Provide landlord tenant mediation as needed. 11. Work with landlords to increase opportunities for persons with disabilities. 12. Complete reporting as requested. Prepare monthly time certifications and other reporting as needed. 13. Function as part of LVCIL Team and Core & Housing Team and attend related meetings. 14. Maintain a flexible schedule to meet all consumer, program, and team needs. 15. Maintain a detailed daily schedule in Outlook. Communicate schedule changes to supervisor and always assure consumer continuity of services. 16. Record time and all consumer contacts in CILSuite daily and provide time sheets every other week as scheduled. LVCIL Team Support 1. Serve as a member of full LVCIL Team - attend required meetings. 2. Provide back up to consumers with respect to absence of other team members as requested, always ensure provision of quality services. 3. Assist in smooth agency operations; assist as needed with special events. General 1. Responsible for other duties as specified by the Core leadership team, Assistant Executive Director, or Executive Director. 2. Accept responsibility as directed for other miscellaneous agency needs. 3. Off-site travel as needed via independent, readily available means of transportation, with proof of valid driver's license, insurance and registration provided at least annually. IV. Accountability The Community Support Coordinator is responsible to, and will be evaluated by the Supervisor of Core & Housing Services, Core & Housing Services Program Manager, or, in their absence, the Assistant Executive Director and Executive Director.

Pay:

$18.50 per hour Expected hours: 25.0 per week
