Job Description

Certified Social Services The Certified Social Services supports residents at Mission Pines by assisting with their physical care needs while also providing emotional, social, and psychosocial support. This hybrid role combines hands-on caregiving with responsibilities that promote resident well-being, dignity, and quality of life. The position works closely with the interdisciplinary team to ensure individualized care and positive resident experiences.
Key Responsibilities Certified Social Services Duties:
Assist the Social Services Director in identifying and addressing the psychosocial, emotional, and discharge planning needs of residents. Provide support to residents and families during admissions, care conferences, and transitions of care. Help coordinate resources, services, and referrals to community agencies as needed. Document social service interactions and maintain required records in compliance with facility, state, and federal regulations. Assist in organizing resident programs, support groups, and activities that enhance emotional well-being. Act as a liaison between residents, families, and staff to promote positive communication.
CNA Duties:
Provide direct resident care, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and mobility. Take and record vital signs, report changes in resident condition to nursing staff. Ensure resident comfort, safety, and dignity at all times. Assist residents with positioning, transfers, and use of adaptive equipment. Support residents during mealtimes, activities, and therapy sessions. Maintain accurate documentation of care provided. Qualifications Long term care experience Social Services Certification High school diploma or equivalent required. Current certification as a Certified Nursing Assistant (CNA) in good standing with the state. Experience or training in social services, counseling, psychology, or related field preferred. Strong interpersonal and communication skills; ability to work compassionately with residents and families. Organizational skills with attention to detail and documentation. Ability to maintain confidentiality and comply with HIPAA regulations. Work Environment Skilled nursing facility environment with interaction among residents, families, staff, and community resources. May require standing, walking, lifting, and assisting residents throughout the shift.
