Full Job Description Role and Responsibilities:

Assist children with developmental disabilities in building on their strengths Help families remain safely together in the family home Work in an environment convenient to the child and family Link, monitor, and coordinate services Develop and implement Person-Centered Plans for assigned individuals Advocate effectively for individuals served Monitor and document outcomes, revising plans as needed Support culturally competent, recovery-based practices

Qualifications:
