Job Description
This is an excellent opportunity for professionals with backgrounds in developmental disabilities, ABA, CSOC, social work, case management, or behavioral health who are looking to make a lasting impact. Responsibilities Coordinate and monitor services for individuals with intellectual and developmental disabilities. Conduct monthly contacts, home visits, and person-centered assessments. Develop and maintain individualized service plans. Collaborate with families, providers, and interdisciplinary teams. Connect individuals with community resources and available supports. Maintain accurate and timely documentation in accordance with NJ DDD requirements. Advocate for individuals to ensure they receive appropriate services and supports. QualificationsRequired Minimum 1 year of experience working with individuals with developmental disabilities (DD). Bachelor's degree in Social Work, Psychology, Human Services, Special Education, Counseling, or a related field (or equivalent experience if permitted by NJ requirements). Excellent communication, organizational, and time management skills. Reliable transportation and willingness to travel throughout New Jersey. Ability to work independently while managing a caseload. Preferred Experience in Applied Behavior Analysis (ABA) . Experience working within CSOC/PerformCare . Background in social work, case management, behavioral health, or related human services . Knowledge of NJ DDD programs and community resources.
