Developing, implementing, and monitoring individualized service plans. Conducting assessments and documenting participant progress. Connecting individuals and families with community resources and service providers. Coordinating with healthcare providers, social service agencies, schools, and other professionals. Educating participants about available service options so they can make informed decisions. Monitoring service effectiveness and recommending changes as participants' needs evolve. Maintaining accurate documentation while ensuring compliance with Medicaid Waiver requirements. Building trusting relationships with participants, families, and community partners. Advocating for participants to ensure they receive the services and support they need. Qualifications Required Bachelor's degree in: Psychology Sociology Social Work Criminal Justice Education Human Services Or another closely related human services field Preferred Experience in case management, care coordination, or social services Experience working with individuals with developmental disabilities Knowledge of Medicaid Waiver services is a plus (training provided) New graduates with a bachelor's degree in a qualifying human services field are encouraged to apply.
