Support Coordinator Assistant B3 Community Alliance (B3CA)
Location:
Lake Charles / Region 5 -
Southwest Louisiana Schedule:
Full-Time | Monday-Friday | 8:00 AM-5:00
PM Starting Pay:
$15.00-$16.00 per hour

In-Office & Community-Based/Field Support Job Overview B3 Community Alliance is seeking a dependable, organized, and compassionate Support Coordinator Assistant (SC Assistant) to support our Support Coordinators in providing services to individuals participating in Louisiana Medicaid Home and Community-Based Services (HCBS) programs. This is an excellent opportunity for an individual currently pursuing a degree in social work, psychology, sociology, counseling, human services, rehabilitation services, or another applicable human-services field who wants hands-on experience working with individuals with disabilities, older adults, families, and community service providers. The SC Assistant works under the direction of B3CA Support Coordinators and supervisory staff and does not independently manage a Support Coordination caseload or perform functions that require a qualified Support Coordinator. Essential Responsibilities Assist Support Coordinators with participant intake, scheduling, service coordination follow-up, record maintenance, data entry, and required documentation . Assist with scheduling participant contacts, home visits, planning meetings, assessments, and other appointments. Contact participants, families, providers, and authorized representatives for approved follow-up activities as directed by the assigned SC. Assist SCs with gathering documents and information needed for participant records, Plans of Care, revisions, and service coordination activities. Track outstanding documentation, deadlines, provider information, and follow-up items. Assist with maintaining accurate participant records in B3CA and authorized state systems. Assist with uploading, organizing, scanning, and filing participant documentation. Help prepare participant files and documentation for quality reviews, audits, supervision, and monitoring . Assist SCs with locating community resources and obtaining information from providers. Communicate outstanding issues and participant/provider concerns promptly to the assigned Support Coordinator or supervisor. Accompany Support Coordinators on approved field activities, meetings, or home visits when appropriate. Maintain confidentiality and comply with HIPAA, Louisiana Medicaid, LDH, and B3CA policies and procedures . Participate in required staff meetings, supervision, orientation, and training. Perform other administrative and Support Coordination assistance duties as assigned and permitted within the scope of the position. Minimum Qualifications Currently enrolled in and actively pursuing a college degree in Social Work, Psychology, Sociology, Counseling, Human Services, Rehabilitation Services, or another applicable human-services-related field . Interest in developing a career in Support Coordination, case management, social services, disability services, or a related field. Strong written and verbal communication skills. Strong organizational and time-management skills. Ability to maintain accurate and confidential records. Basic computer skills and ability to learn electronic case-management/documentation systems. Ability to communicate professionally with participants, families, providers, and staff. Must successfully complete required background checks, exclusion checks, onboarding, and training requirements . Additional Requirements Valid driver's license, automobile insurance, and reliable transportation . Ability to travel throughout Region 5 when required. Ability to work Monday-Friday, 8:00 AM-5:00 PM . Ability to work primarily in the office while providing community/field support as assigned. Must maintain professional boundaries and participant confidentiality. Must work under the direction and oversight of B3CA supervisory staff and qualified Support Coordinators. Starting Pay:
$15.00-$16.00/hour
