Entry level position . Madison County 310 Board is hiring for a full-time Support Coordinator to coordinate services for individuals with intellectual disabilities in Madison County. The ideal candidate must be detailed oriented and possess excellent organizational skills. The candidate must also have good written and oral communication skills, be able to work with providers and families in a professional manner, and have the ability to advocate for the individuals we serve. Job Overview:
Assisting Medicaid eligible individuals with intellectual disabilities in gaining access to needed medical, social, educational, social/psychological, physical, and residential supports, as well as other identified services. Provide at a minimum, the following direct case management activities to adults with intellectual disabilities including Needs Assessment, Case Planning, Service Arrangement, Social Support, Reassessment/follow-up, and Monitoring.
Minimum Qualifications:
BA/BS degree in human services field from an accredited college or university A valid driver's license and good driving record
